Disclaimer/Waiver Release

You knowingly and freely assume all such risk, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility. I for myself and on behalf of my heirs, assign, personal representatives and next of Ken, hereby release and hold harmless, Jamal willis and his staff, officers, officials, agents/ and or employees, other participants, sponsoring agencies and if applicable owners and owners, lessors of premise used to conduct this event (release), with respect to any and all unjustly, disability, death, or loss or damage to personal property, wether arising from the negligence of the release or otherwise. I have read this release of liability and assumption of risk agreement, fully understand it’s terms, understand that I have given up substantial rights, by signing by paying registration fee, and I sign it freely and voluntarily without any inducement. This is to certify that I, as a parent, Coach, trainer, athlete with legal responsibility for myself and my athletes, do agree to his or her release as provided above, of all releases. I release and agree to indemnify and hold harmless the release from any and all liabilities incidents to myself or my athletes, or participants in my programs as provided above even if arises from their negligence.
